Now a days Hemiplegia / paralysis has become a common condition. The cause can be Stroke or Spinal cord injury or traumatic skull injury. In my practice, i have seen most of the rehabilitation programms are gait (walking pattern) oriented in my area. But we need to work out the upper limbs too, so this article comprises of the upper limb exercises in hemiplegia. After getting an admission for a Stroke/ CVA , we have to wait for the vitals (Blood presure, pulse etc.) to get stable. It may take a time period of 7 days to 1 month. Most of the people have habit of sleeping in the side-lying position; they say “I can’t go to sleep without side-lying”. But do you know; it can be hazardous to your Shoulder? Let me explain! How? Lateral sleeping / side-lying sleeping can give rise to shoulder impingement symptoms. People on the risk are: 1) Those who have kyphotic posture (forward bent posture/ slouch kind of posture). 2) Diabetic patients. 3) People with vitamin B12 and calcium deficiency. 4) Those who get recurrent shoulder dislocation/ laxity. 5) Hemiplegics/ paralysis cases. 6) A patient with collie’s fracture (wrist fracture) has a tendency to sleep in one side. Complaints of Overnight Shoulder stiffness..

Once a guy came to me with a complain of severe pain & stiffness in his Right shoulder. He was unable to reach his hand to the backpocket too. After a talk i got to know, it was a overnight problem. I examined all of the muscles & joint ranges. All ranges were full but some manual teats were positive for Supraspinatus & infraspinatus muscle. His Infraspinatus muscle seemd to be very weak. * Always remember isolated (only one) muscle weakness is indicative of some kind of neuropathy. And in case of shoulder, Suprascapular neuropathy is the commonest one. The neuropathy can be diabetic, traumatic or diet related . In case of this guy previous 2 reasons were not there. Deficiency of B12 can give rise to neuropathy. Ask the patient diet related quostions like veg/nonveg? does he consums milk? etc. nonveg & milk are good sources of Vitamin B12.
